- Visit FieldPrint and select Schedule an Appointment got Live Scan Fingerprints.
- Use the Filedprint Fingerprint code: FPHIBrdNursing
- Schedule an appointment near you and pay the $52 fee.
4.) Run and Upload a NPDB Self-Query Report
- Visit the National Practitioners Data Bank Self Query site
- Once complete, upload the PDF report to your online portal
